VOLUNTARY HEALTH SCHEME OF IDBI BANK LTD FOR
RETIREES (NO NEW MEMBERS ARE TAKEN NOW)
PRELIMINARY : This column provides information useful to the employees / retired employees.
PROCEDURE FOR CASHLESS HOSPITALISATION FOR EMPLOYEES AND RETIREES :
As of now, one who needs cashless admission in a network hospital and avail of the Bank's scheme has to arrange to obtain the following :
1. The name of the hospital approved by the Bank where he can seek admission.
2. An estimate of the expenditure that could be incurrred on admssion and treatment.
3. A Cheque / DD for 25% of the estimated amount drawn in favour of the bank.
4. Keep the tel nos of medical section and BMO"s dispensary ready.
The retiree has to get a letter from the bank addressed to the hospital. The estimated amount given by the hospital, the cheque/dd bearing 25% of the estimate and a form kept in the dispensary of the BMO attached to the office has to be filled in, signed and handed over to the Medical Section. The letter addressed to the hospital from the Medical Section has to be handed over to the hospital.
Alternately, there is also an option to get admitted into any recognised hospital, pay for the treatment and claim reimbursement for the treatment availed from the Bank under the VHS Scheme. The Bill will be settled as per extant norms framed by the Bank.
MEDICAL REIMBURSEMENT ISSUES :
The Medical Section is shifted to CBD Belapur.
All Medical claims / bills should be hand delivered or sent by courier / speed post to :

Bills should be tied with a string to the application form and not just stapled. Stapled bills can get separated. Keep a copy of all bills as well as the application form sent to Medical Section. Make a note of all reimbursements in a diary by recording the date and amount credited in your SB SAvings Account. As the VHS domciliciary amount accrues every fiscal year, the amount settled minus the amount accrued and carried forward (per 3 years) would be the VHS balance. Do not forget to write your employee code and mobile no. in the VHS Claim Form. Supercribe the cover with the words : "VHS Bills/Claims". The Cover should be large enough to hold the enclosures and leave some free space.

PHARMACIST AND DOCTOR ON 2ND FLOOR IN HO. Those VHS Members who visit BMO should quote their employee code and based on that the case book no. and note down this number with them for quoting whenever required.
DISPENSARY / PHARMACIST : TEL NO. 3442
Retd employees should write their VHS booklet roll no. in the dispensary to enable retrieval of the booklet for Doctor consultation.
The latest combined form for reimbursement of hospitalisation / non-hospitalisation expenses is enclosed.
The annual limit for claim under VHS is as follows :
Gr.’F’ and above : Rs.17500/-
Gr.’D’ & ‘E’ : Rs.15000 /-
Gr.’A’,’B’ and ‘C" - Rs.12500,
Class III : Rs.8000/- and
Class IV : Rs.6000/-.
Post retirement, during the lifetime of the employee, the current limits are : EDs who retired after 2007 unlimited free treatment subject to a limit of 75% of the claimed amount. All those who retired as Officers : Rs.9,00,000 and others : Rs.8,00,000/-. All claims need to be supported by bills / receipts; if bank stops asking for these either in annual VHS claims or otherwise, you would need to maintain these bills / receipts for tax purposes.
VERY IMPORTANT : Claimants should fill in the form carefully giving all necessary details and mention their correct employee code with the check digit, landline / mobile tel no., email id (if available) and their Bank A/c.Number. If the employee code or the Bank A/c. no. is not correct & if there is some query / clarification to be obtained Medical Section will find it difficult to process the claim in the absence of the contact details of the claimant.
